Dr. Wallace was educated at the University of Illinois. Earning his BS in Zoology in 1966, then his DDS in 1970. 

Oral and Maxillofacial Surgery Internship at Long Island Jewish Medical Center/Queens Hospital Center Affiliation, in JamaicaQueens, New York from 1970 to 1971. Then his Oral and Maxillofacial Surgery Residency and Chief Residency, Long Island Jewish Medical Center/Queens Hospital Center Affiliation, Jamaica, Queens, New York, from 1971 to 1973. 

Instructor- Histology and Pathology, University of Illinois College of Dentistry  

Attending Oral Surgeon, Michael Reese Hospital and Medical Center Postgraduate Residency Program.  Responsibilities included didactic and clinical education of residents and interns in both inpatient and outpatient surgery. 

Director of Oral Surgery, St. Elizabeth’s Hospital, Chicago. Responsibilities included education of medical and dental students, coordination of Continuing Education for the professional staff, and intramural practice. 

Presented multiple seminars on orthognathic surgery, dental implant surgery, dental insurance issues and various other surgical topics on the local and national level. 

Dr. Wallace is a member of the American Dental Association, American Association of Dental Consultants, Illinois State Dental Society, Chicago Dental Society, American Association of Oral and Maxillofacial Surgeons, Illinois State Society of Oral and Maxillofacial Surgeons, and the American Academy of Implant Dentistry.
